# AHRMM/AHA Supply Chain Resource Council Report (September 2026)

by Lauren Palmer

 September 22, 2026

The BD ChloraPrep Convenience Kits recall is creating sourcing and substitution challenges for health systems.

BD is conducting a recall of specific lots of ChloraPrep applicators due to a potential breach of sterility, including product that may have an open or incomplete seal on the applicator packaging. The recall extends to convenience kits containing the affected applicators.

The AHRMM/AHA Supply Chain Resource Council’s September report (see below) noted operational impacts on health systems related to the recall. One health system reported difficulty sourcing product and, more significantly, difficulty identifying clinically acceptable substitutes because appropriate alternatives vary by clinical application. The organization is launching a cross-regional work group to compare best practices, determine substitute selection by use case and define the clinical education required to deploy alternatives. A second health system reported similar sourcing challenges.

The report noted an otherwise “comparatively stable” supply environment. No new shortages were reported, though recall activity was elevated, with the following recalls shared. No patient care impact was reported from the recalls discussed.

- Baxter: Cefazolin in Dextrose Injection: Voluntary nationwide recall of one lot due to a report of particulate matter found in the solution identified as cardboard.
- B. Braun: 0.9% Sodium Chloride Injection: Recall due to presence of particulate matter identified as iron oxide, inorganic, cellulose or polystyrene particulate matter.
- American Regent: Epinephrine Injection: Recall of three lots due to the presence of particulate matter identified as Nylon, Cellulosic, Acrylic, Polyethylene and Glass.
- BD ChloraPrep applicators and convenience kits: Recall of specific lots due to potential breach of sterility in the applicator packaging.
